District nursing and end-of-life care

<p>It is currently, widely believed that the majority of end-of-life patients would prefer to die at home given the choice. Care of the dying patient in the community often falls to the district nurse. However, without a clear ‘care of the dying’ pathway in place, district nurses can find it difficult to deliver adequate end-of-care. This article describes the development of such a pathway by teams involved in end-of-life care.</p>
