<p>DURING INFORMAL conversations with nurses on a transitional care unit at a local hospital, it became apparent that although there was space on nursing assessment forms for 'biography', they were unclear how or why this should be used. Nursing/residential home staff had also expressed a desire to know more about patients.</p>
