The discharge process for palliative care patients
<p>In 1993, following implementation of the NHS and Community Care Act (Audit Commission 1990), the discharge of palliative care patients into the community became more complex. All providers of social and health care are now required to communicate, coordinate and co-operate with each other to formulate comprehensive and individualised care packages (McWilliam and Wong 1994).</p>
