<p>Diabetes is the most common cause of end-stage renal disease, and the number of renal patients with diabetes-related problems is increasing. Anecdotal evidence from a 27-bed nephrology ward highlighted an increase in diabetic foot complications often leading to amputation. Preventive measures using an assessment tool have been initiated to improve outcomes for renal patients with diabetes. The aim of the tool is to highlight problems or those at risk of developing problems.</p>
