<p>Conscious of the implications of the Patient’s Charter and the tensions introduced by the internal market, nurses in the stroke rehabilitation unit (SRU) proposed to improve nursing documentation. This in turn created a catalyst for radical change to all the records used in the unit. The outcome of the exercise was a document which could be used by all members of the multidisciplinary team to record the care of patients in the unit.</p>
