Patient death: prescription charts ‘not completed on 15 days’
Coroner tells Worcestershire Royal Hospital it must urgently improve its nurses’ awareness of their professional duty to complete important patient documentation, after record-keeping failure in case of patient who died following a pulmonary embolism. An inquest into Vauna Leeming's death heard the patient’s prescription chart for compression stockings was not completed on 15 out of 46 days, including five consecutive days. In a prevention of future deaths report to Worcestershire Acute Hospitals NHS Trust (WAHT), the coroner says this suggests either the nurses’ lack understanding of their professional duty or that such practice is commonplace.
