An inquest into Vauna Leeming's death heard the patient’s prescription chart for compression stockings was not completed on 15 out of 46 days, including five consecutive days. In a prevention of future deaths report to Worcestershire Acute Hospitals NHS Trust (WAHT), the coroner says this suggests either the nurses’ lack understanding of their professional duty or that such practice is commonplace.
