A coroner report found that Colin Colley was at high risk of falls and experienced a fatal brain bleed following a fall at St David’s Hospital, Cardiff. Coroner says evidence suggests hospital staff lacked confidence in use of falls risk assessments, and use and importance of enhanced supervision, and advises that more training is provided and refreshed frequently to prevent risk of future deaths occurring
