Prostate cancer is the most common cancer in men in the UK (Office for National Statistics 2013) and more men are now surviving because of improved treatments (Cancer Research UK 2015). A national initiative to provide follow-up pathways as alternatives to traditional hospital outpatient care aims to improve the quality of care while freeing up capacity in secondary care for diagnosis and treatment of new patients. The project described here aimed to test a primary care-led pathway for prostate cancer follow-up. This article outlines the rationale for developing the pathway, provides an overview of the pathway and describes the tools developed to support clinicians in managing prostate cancer as a long-term condition in the primary care setting.
