Developing an anticipatory healthcare calendar
<p>This article discusses the problems faced by people with learning disabilities who have complex health needs and how a staff team developed a unique tool for improving health surveillance and access to health care. It describes the design and piloting of the tool, and makes recommendations regarding the improvement of anticipatory health care for vulnerable patients.</p>
