Developing person-centred planning in dementia care
<p>This article provides an overview of the current literature on person-centred planning, health action planning and associated concepts such as facilitation and advocacy. It explores how flexible approaches to person-centred planning and health action planning were introduced and adapted to meet the individual daily changing needs of Anna, an older person with Down syndrome and dementia.</p>
