<p>The author describes how the Gloucestershire reasonable adjustment outcome tool was developed and what it is intended to achieve. Along with details of how the tool was implemented, its structure and content are also described to aid effective use. The author hopes that others will use the concept and the design of this approach to enhance the development of reasonable adjustments and access to mainstream services in their areas.</p>
